Directions for New Patient Health History

1. Your must have Adobe Reader 7 or higher installed on your computer to view and process this document. If you are unable to view this document, click here to download and install the latest version of Adobe Reader.
2. 1. Right click the green arrow under the "PDF" column in the "New Patient Health History".
   2. Select "Save Target As".

3. Save the file on a easy to find location on your computer, and open it at that location.
4. Fill out all of the fields in the form.
5. After completing the form, save the PDF file again by selecting: File -> Save.
6. In your e-mail client, attach the PDF file to an e-mail and send it to:
