Instructions to email forms:
1) Open appropriate document in Word format.
2) Fill out form completely.
3) Save your completed form to your desktop. (This will retain a copy for you) 
4) To email, simply type in our address info@paincareboise.com in your “To” browser.  Please note in the “Subject” space that these are “Patient forms for (your name)”. 
5) Attach your saved form(s) from your desktop to your email.
6) Send
